Reports – Project Grants
In accordance with the grant agreement for project support grants, grantees should use these forms to report on progress toward the stated goals and objectives of your project and to provide details on expenditures against approved budgets, other funding sources, and other financial information. The narrative and financial reports must be submitted together.
Reports – General Purposes Grants
In accordance with the grant agreement for general purposes support grants, grantees should use the narrative report form to report on your organization's major activities during the reporting period. For the financial report requirement, you should submit a copy of your organization’s financial statements that include the reporting period. The narrative report and financial statements must be submitted together.
Affidavit Packet for Non-U.S. Grant Applicants
This affidavit packet is required to be completed by all non-U.S. grant applicants. The packet contains instructions and the necessary forms to allow the Mott Foundation to determine whether your organization qualifies as a public charity in the U.S.
Affidavit Update Packet for Non-U.S. Grantees
This affidavit update packet is required to be completed by all non-U.S. grantees approximately every two years to ensure that your organization continues to qualify as a public charity. The packet contains instructions and the necessary forms to update your organization’s original Affidavit to the Mott Foundation.
Redistribution Statement for Project Grants
Certain grantees (other private foundations or private foundation equivalents) should use this form to certify how project support funding from the Mott Foundation was spent. The commitment letter will inform the grantee if and when this form is required.
Redistribution Statement for General Purposes Grants
Certain grantees (other private foundations or private foundation equivalents) should use this form to certify how general purposes funding from the Mott Foundation was spent. The commitment letter will inform the grantee if and when this form is required.
Request for Wire Transfer of Funds
This form contains all the required information for the Foundation to make a wire payment. Please note that this payment option is available only to overseas grantees.
