Progress Reports — Project Grants
In accordance with the grant agreement for project support grants, grantees must submit a narrative report to describe progress toward the stated goals and objectives of your project, and a financial report to provide details on expenditures against approved budgets, identify other funding sources, and explain budget variances. The narrative and financial report forms below can be used for these requirements. Both reports must be submitted together.
Progress Reports — General Purposes Grants
In accordance with the grant agreement for general purposes support grants, grantees must submit a narrative report to describe your organization’s major activities during the reporting period. The narrative report form below can be used for this requirement. For the financial report requirement, you should submit a copy of your organization’s financial statements that include the reporting period. A narrative report and financial statements must be submitted together.
Affidavit Packet for Non-U.S. Grant Applicants
This affidavit packet is required to be submitted by all non-U.S. grant applicants with your first application, as well as every 6 years after that point. The packet contains instructions and the necessary forms to allow the Mott Foundation to determine whether your organization qualifies as a public charity in the U.S.
Please note that the Affidavit Update Packet below should be used during the time between the original affidavit submission and the 6 year affidavit submission.

Affidavit Update Packet for Non-U.S. Grantees
This affidavit update packet is required to be completed by all non-U.S. grantees approximately every two years to ensure that your organization continues to qualify as a public charity (see instructions regarding the 6 year requirement above). The packet contains instructions and the necessary forms to update your organization’s original Affidavit to the Mott Foundation.

Redistribution Statement for Project Grants
Certain grantees (other private foundations or private foundation equivalents) should use this form to certify how project support funding from the Mott Foundation was spent. The commitment letter will inform the grantee if and when this form is required.
Redistribution Statement for General Purposes Grants
Certain grantees (other private foundations or private foundation equivalents) should use this form to certify how general purposes funding from the Mott Foundation was spent. The commitment letter will inform the grantee if and when this form is required.
Wire Transfer of Funds Form — General
This form contains all the required information for the Foundation to make a wire payment. Please note that this payment option is available only to overseas grantees.
Wire Transfer of Funds Form — Ukraine
This form contains all the required information for the Foundation to make a wire payment to grantees located in Ukraine.
Grant Certification and Summary — Ukraine-based Grantees
Grantees located in Ukraine should use this form to certify that they are in compliance with the U.S. Department of Treasury’s guidelines. The commitment letter will inform the grantee if and when this form is required.
